Dear Applicant:
We are excited that you have shown interest in applying for employment at Central Arkansas Veterans Healthcare System. Enclosed is an
application and information packet.
Required Documents:
Completed application
Resume, (experienced nurse), Complete work history required (month and year)
Nursing School Transcript(s) - official copy mailed directly from your school(s) to the Nurse Recruitment Office. (Please order
immediately)
Continuing Education Certificates, if applicable
Nurse License(s), all states, active and inactive
Three reference forms, one of which MUST BE from a current clinical supervisor or most recent
clinical supervisor (new graduate, forms should be completed by clinical instructors). These forms should be faxed or mailed
directly to the Nurse Recruitment Office.
Copies of current evaluations
Current pay stub (if available)
DD Form 214 required for veterans
Submission of data into VetPro (after the interview portion is complete).
